CUSTOM ORTHOTIC PROCESS:
STAGE 1: “BEFORE” BOOKING YOUR ORTHOTIC ASSESSMENT
Please check your Extended Health Insurance Policy / talk to an adjustor to see what type of coverage you have for Orthotics. Not all policies and/or Insurance Companies are the same.
QUESTIONS FOR YOUR INSURER:
» What type of referral is required? A referral from:
- Your family physician?
- A podiatrist? (We will be able to provide you with one but let us know)
- Both the family physician and the podiatrist?
» How much Coverage do you have?
» Does your insurer need us to do foam casting in addition to our other assessment procedures.
» Is a predetermination letter required to ensure your Insurer will provide payment for your orthotics?
STAGE 2: BOOKING YOUR ORTHOTIC ASSESSMENT
» When you call us for an appointment, we will book a 1 hour appointment with our Registered Physiotherapist for a Gait Scan and/or Foam Casting and Lower Extremity Biomechanical Assessment.
» We require a deposit of $250.00 payable at time of assessment. This is required before we place the order for your Orthotics.
» It takes approximately 1 to 2 weeks for your orthotics to arrive.
» When your orthotics arrive we will call and book your 2nd appointment (up to a ½ hour). You will receive your orthotics and instructions and at this time the balance of $250.00 is due.
» We will provide you with the necessary invoice/letters/documents to send in to your insurance company for re-imbursement.
» Total price of Orthotics: $500.00.
