CUSTOM ORTHOTIC PROCESS:
Stage 1: Before Your Orthotic Assessment
» Check your Private Insurance Policy to see what type of coverage you have for
Orthotics. Not all policies and/or Insurance Companies are the same.
» Bring a copy of your Private Insurance Policy showing your Orthotic coverage. This ensures that we will provide you with all the proper documentation needed for re-imbursement of your orthotics.
» Things to look for when checking Insurance Policy:
• What type of referral is required? A referral from your family physician is usually required.
• How much Coverage do you have?
• Do they require a foam casting and/or letter from a Podiatrist? We will provide you with one.
Stage 2: Booking Your Orthotic Assessment
» When you call us for an appointment, we will book a 1-hour appointment with one of our Registered Physiotherapists for a Gait Scan and/or Foam Casting and Lower Extremity Biomechanical Assessment.
» We require a deposit of $250.00 payable at time of assessment. This is required before we place an order for your Orthotics.
» It takes approximately 1 to 2 weeks for your orthotics to arrive.
» When your orthotics arrive, we will call and book a second appointment (up to a ½ hour). You will receive your orthotics and wearing instructions. At this time, a second payment of $250.00 is due. The total cost of your orthotics is $500.
» We will then provide you with the necessary invoice/letters/documents to send in to your insurance company for re-imbursement.
